            Scholarship Application Checklist

 In order to be considered for a  scholarship you must complete and return this check list and the attached application, postmarked no later than the end of the first quarter of the year.  You must also include an official High School transcript, proof of financial need using one of the forms indicated below, a copy of your SAT or ACT test scores and a 500 word essay on one of the indicated subjects.

 Please indicate which documents you are including with your submission:

You must include each of the following documents:

ÿ       Scholarship check list

ÿ       Scholarship application

ÿ       Official High School Transcript

You must also include one of the following documents:

ÿ       Copy of SAT Scores

ÿ       Copy of ACT Composite Scores

You must also include at least one of the following financial documents:

ÿ       Copy of Student Aid Report

ÿ       Copy of parent and applicant currently filed Federal Income Tax Returns

ÿ       Copy of University or College Specific Financial aid report ( only for schools that have a unique financial aid process)

 

You must also include a 500-word (maximum) signed essay on one of the following topics:

ÿ        “The importance of Celebrating Our Italian Heritage in Modern Day America”

ÿ       “Who  You Believe is the Most Influential Italian-American of the 20^th Century and Why”
