Dues are $40.00, per year.  As well as an initiation fee of $20.00 is also required of new members. A total initial amount of $60.00 may accompany this form, or be paid at the time of completion of an official application form.

You will be contacted and given an official Order Sons Of Italy (O.S.I.A) membership form approved by our National Lodge and additional membership information and requirements.
